Does this web design contract include notification rules?

Yes: a notices clause is included in the "General" section. The notices clause specifies the method by which notice must be given (e.g. by first class post, fax or email) and the address or number to which the notice must be sent. The notification addresses/numbers may of course be updated upon written notice. There is also a deemed service clause, the purpose of which is to deem that notice is successfully served at a specified time where a party gives notice in accordance with the notices clause.

What sort of problem or error with a site delivered to a customer will lead to the designer being in breach of contract?

That depends upon the specific circumstances of the case. The concept of a "defect" is defined in the contract to mean any defect, error or bug having a material adverse effect on the appearance, operation of functionality of the website. However, certain specified types of defect are excluded: those cause by the customer, and those arising out of an incompatibility between the website and software that the parties did not specifically agree would be compatible at the outset.
